The Central Florida Collaborative has launched the 2025 Community Health Needs Assessment (CHNA) Community Survey for residents of Orange, Osceola, Lake, and Seminole counties.
The survey is available in five languages: English, Spanish, Chinese, Haitian Creole and Portuguese.
We invite you to participate in the process by helping to identify our community’s current strengths, resources, needs, and service gaps!
